BrightStar Care is looking for a dedicated Physical Therapist to work 23 days per week , providing personalized in-home care that helps clients remain safe, healthy, and independent in their own homes. As a home health agency, we are proud to partner with the Veteran's Administration to serve Veterans across the Twin Cities and surrounding communities, and we are currently seeking a PT to join our team and support clients in the St. Paul and North Metro areas.

Assessment, planning, implementation, and evaluation of all physical therapy modalities to patients.All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization.Improves or minimizes residual physical disabilities of the patient.Returns the individual to optimum and productive level within patients capabilities.Participates with all other home care personnel in patient care planning.Takes initial history and makes initial evaluation.Consults with physicians regarding patients progressParticipates in quality assessment and performance improvement activities as assigned.Prepares clinical and progress notes.Helps develop the plan of care and revise, as necessary.Consults with family and agency personnel.Ability to do extensive bending, lifting and standing on a regular basis.Requirements :
A graduate of a PT Program recognized by the American Physical Therapy Association or other National accepted Council or AssociationAn active license to practice Physical Therapy in MNA minimum of four (4) years experience as a Physical Therapist, home care experience a plusMust have criminal background checkMust have current CPR certificationMust have active driver's license and clear driving recordComputer skills.Ability to exercise initiative and independent judgmentAs a Physical Therapist at BrightStar Care, you will play a critical role in shaping our clients' care plans and enhancing patient outcomes.
