LCSW Care Navigator

A company is looking for an LCSW Care Navigator (1099 / Remote).

Key Responsibilities

Establish meaningful relationships with members and their families, grounded in empathy and trust

Perform assessments and develop personalized care plans that address medical, emotional, and social needs

Coordinate care across providers and systems, helping members receive the right care at the right time

Required Qualifications

Active LCSW license in good standing

Specialization in care coordination, case management, or clinical advocacy, preferably with Medicare or older adult populations

Deep understanding of Medicare, social drivers of health, and complex care systems

Experience and comfort with using technology and telehealth platforms
