Description : Job Summary :

The Qualified Intellectual Disabilities Professional (QIDP) is responsible for coordinating and overseeing the delivery of services and supports for individuals with intellectual and developmental disabilities. The QIDP ensures all supports are person-centered and compliant with federal and state regulations, including those of the Illinois Department of Human Services (IDHS), Department of Public Health (IDPH), and Centers for Medicare and Medicaid Services (CMS).

Key Responsibilities :

• Serve as the primary case manager for individuals on assigned caseload.
• Develop, implement, monitor, and revise Individual Service Plans (ISPs) in collaboration with the interdisciplinary team, individuals served, and guardians.
• Ensure services provided are person-centered, promote independence, and reflect the individual’s goals and choices.
• Conduct quarterly reviews, assessments, and annual evaluations in accordance with DHS Rule 115 and 116.
• Monitor and document progress on ISP goals and ensure timely submission of required reports and documentation.
• Coordinate and facilitate interdisciplinary team (IDT) meetings.
• Advocate for individuals’ rights, preferences, and needs.
• Ensure compliance with health, safety, and regulatory requirements.
• Train and support Direct Support Professionals (DSPs) to implement individual plans effectively.
• Monitor program environments to ensure quality, dignity, and a homelike atmosphere.
• Participate in audits, site visits, and licensing reviews as required.
• Maintain accurate and complete documentation in accordance with agency policy and regulatory standards.
