Responsibilities

Assist in the delivery of primary health care services and patient care management.

Demonstrates competence in the skills necessary to carry out assigned duties.

Administer medications by all routes.

Apply Nursing Process (Assessment, Diagnosis, Planning, Intervention and Evaluation – within licensure guidelines)

Act as a liaison with patients, family and providers consistently demonstrating smooth and efficient telephone communication when covering provider call and scheduling patient appointments.

Obtain complete history regarding health problems according to protocols.

Document accurate information regarding health concerns and / or condition from patient or family member.

Telephone Triage by providing and documenting advice for home care of illness / condition; conducting follow-up phone calls regarding patient's health and progress as necessary.

Ability to assess urgency of condition for immediate attention regarding illness related calls and assign priority.

Provide patient education regarding health maintenance and medical illnesses following accepted Teach-back method per protocols and guidelines.

Provide appropriate and timely documentation in the patient’s electronic health record using standardized workflows and processes.

Qualifications

Graduate of an accredited program for Registered Nurses.

Current license to practice nursing in the State of Iowa / Illinois for Registered Nurse.

Mandatory Reporter certification

CPR certification.

Previous clinical experience in a medical office preferred.
