JOB SUMMARY\n\nThe RN Promotes and restores patient's health by completing the nursing process; collaborating with physicians and multidisciplinary team members and providing physical and psychological support to patients, friends and families. Assesses wounds, creates wound care treatment plans, makes recommendations, provides education and documents plan of care.\n\nESSENTIAL FUNCTIONS\n\nPatient Care\n\n
- Obtains patient consent for care and ensures patient confidentiality. Obtains and documents health history, information on chief complaint, vital signs and health risk factors. Assesses disease condition using; observational, interviewing interpretation and critical thinking skills in a developmentally appropriate manner.
- Provides patient care based on practice guidelines, standards of care and federal / state laws and regulations. Executes prescribed treatments and medical interventions, administered prescribed medications and monitors and documents treatment progress and patient response. Delegates as allowed under the Nurse Practice Act.\n
- Documents patient assessment and intervention data using established medical record forms / automated systems and documentation practices.\n\nCase Management\n
- Collaborates with other clinics, community resources, providers and staff as needed in a timely and efficient manner. Ensures continuity of care.\n\n
- Educates patient and families about health status, health maintenance and management of acute and chronic conditions.
- Participates in multidisciplinary teams to improve patient care processes and outcomes. Tracks quality assurance data and monitors for acute and chronic care management.\n\nOperating Room / Endoscopy\n
- Assists General Surgeons with procedures in the Operating Room and Endoscopy Room as needed, including Endoscopic Retrograde Cholangiopancreatography (ERCP).\n
- Meets Operating Room standards of sterility; assessed and signed off by Operations Room Charge Nurse.\n
- Assists General Surgeons with scheduling, testing, and patient education with capsule endoscopy and bravo pH reflux testing.\n\nWound Care\n
- Performs wound care at SJH; collaborating with SJH's General Surgeons.\n
- Provides a multidisciplinary approach to the treatment of wounds across a continuum of care and in a number of settings. Provides direct and indirect patient care in the acute care, clinical and long term care settings.\n
- Identifies signs and symptoms that may require referral for surgical debridement or require further attention.\n
- Communicates with Physicians, Directors, Charge Nurses and co-workers, as appropriate, regarding patient condition.\n\nJOB REQUIREMENTS\nMinimum Education\nRequired : Associates Degree in Nursing. Must complete the required Health Stream wound care competencies upon hire.\n\nMinimum Work Experience\nRequired : Must be able to perform specific clinical tasks while providing excellent clinical care. Must possess sound nursing\nknowledge and be able to provide / assist in patient education. Must have sound knowledge of clinic procedures.\nAbility to read / interpret physician notes and orders, analyze options, and provide counsel to patients.\nPreferred : Surgical Assist experience. National Wound Care Certifications.\n\nRequired Licenses : Current RN license with the authority to practice in the State of Wyoming. BLS required at the time of hire. Additional\nACLS certification preferred.\n\nFUNCTIONAL DEMANDS\nWorking Conditions\nHospital based outpatient department, frequent interruptions, sometimes stressful environment. Combination of exam\nrooms and medical offices. Frequent exposure to communicable diseases, toxic substances, ionizing radiation,\nmedicinal preparations and other conditions common to a clinic environment. Must be on-site at the SJH campus or\nother facilities to fulfill the responsibilities of this role.\n\nPhysical Requirements\nStanding and walking for extensive periods of time. Occasional lifting and carrying items weighing up to 10 pounds.\nCorrect vision and hearing within normal range with or without use of corrective devices. Must be able to verbally\ninteract with staff, patients and the public. Manual dexterity for writing and computer use. Requires working in\nemergency situations and may involve irregular work hours including weekends, evenings, holidays and natural\ndisasters.
