• Conduct initial nurse assessments for new clients\n
• Complete follow-up assessments at 30 days, 4 months, and as needed\n
• Review care plans and ensure services meet client needs and Medicaid requirements\n
• Communicate effectively with caregivers, clients, and office staff\n
• Document visits accurately in our CareSmart app (training provided)\n
• Report any significant changes in client condition\n\nRequirements : \n
• Active RN license in good standing with the South Carolina Board of Nursing\n
• Must have reliable transportation and own smartphone\n
• Ability to learn and use our mobile care documentation app\n
• Compassionate, detail-oriented, and professional demeanor\n
