Position Overview
We're seeking a knowledgeable and detail-oriented MDS Coordinator (Registered Nurse) to join our dedicated team. This vital position ensures the accuracy, compliance, and quality of resident assessments and care planning.
As the MDS Coordinator , you'll manage all aspects of the assessment and documentation process, coordinate with interdisciplinary teams, and help ensure that each resident receives personalized, high-quality care aligned with both regulatory standards and clinical excellence.
Key Responsibilities
Assign ICD-10 CM diagnosis codes to residents' medical conditions accurately
Manage Medicare Part A and insurance case management , ensuring timely documentation and compliance
Oversee the resident assessment process - scheduling, completion, accuracy, and transmission of assessments to the Department of Health
Assess residents' needs and develop individualized, resident-driven care plans
Collaborate with facility teams to ensure care plans are effectively implemented and followed
Stay informed on Medicare and Medicaid regulatory updates , adjusting processes as needed for compliance and reimbursement
Initiate and monitor Medicare and Medicaid coverage schedules , including starting or ending coverage as required
Provide leadership and training to staff on assessment documentation and quality care standards

✅ Qualifications
Registered Nurse (RN) in good standing (required)
Associate degree minimum; Bachelor's degree preferred
Minimum 1 year of MDS experience in a skilled nursing or long-term care setting
Resident Assessment Coordinator (RAC-CT) certification preferred
Proficiency with MDS 3.0 and working knowledge of state and federal regulations
Exceptional attention to detail, time management, and leadership skills
Strong communication and teamwork abilities
