Position Summary
Baldwin Family Health Care is expanding clinical pharmacy services! This is a unique opportunity for a pharmacist passionate about working directly with patients in both community and ambulatory care settings. Youll collaborate with an interdisciplinary healthcare team optimize therapies, support chronic disease management, and improve outcomes. This position is evenly split between our family medicine clinic and outpatient pharmacy. Were seeking a candidate excited to contribute to the continued growth of our ambulatory care services.

Key Responsibilities Staff Pharmacist
Deliver dispensing and clinical services, including immunizations, MTM, and patient counselingPromote safe, effective medication use and adherencePartner with providers to address therapy concerns and optimize regimensDocument encounters in pharmacy and clinical systemsMentor and support pharmacy technicians and student pharmacistsKey Responsibilities Clinical Pharmacist
Partner with BFHC providers and staff to deliver evidence-based, cost-effective careConduct patient visits to educate on medications and disease managementManage chronic conditions under collaborative practice agreements (e.g., diabetes, hypertension, hyperlipidemia, anticoagulation, COPD, tobacco cessation)Participate in a multidisciplinary team of physicians, advanced practice providers, nurse care managers, behavioral health specialists, and community health workersRespond to drug information requests, complete comprehensive medication reviews, and resolve therapy-related problemsAssist in developing and evaluating clinical pharmacy servicesQualifications
Doctor of Pharmacy (PharmD) or equivalent degreeCompletion of PGY-1 Pharmacy Residency or comparable ambulatory care experience preferredActive pharmacist and controlled substance license in MichiganStrong interpersonal and communication skillsCommitment to professionalism and working with diverse populationsAbility to thrive in a multidisciplinary team environmentOutpatient or retail pharmacy experience requiredExperience with the 340B Drug Pricing Program preferredFamiliarity with Pioneer Rx and NextGen electronic health record preferredJoin Us
