  • Job Description :
  • Assess, plan, organize, and participate in rehabilitative programs to improve mobility, relieve pain, increase strength, and decrease or prevent deformities.
  • Implement and adjust the physical therapy treatment plan with patient / family input.
  • Provide ongoing patient / family education and training and offer discharge instructions with referrals as appropriate.
  • Act as a resource for staff, families, and physicians about rehabilitation services and equipment.
  • Collaborate with physicians to establish and evaluate treatment programs, plan of care, goals, and discharge plans.
  • Document evaluations, treatment sessions, and any educational or discharge-related activities according to guidelines.
  • Maintain ethical standards, confidentiality, and punctuality.
  • Engage effectively in verbal and written communication with team members.
  • Participate in staff meetings and professional development activities.
  • Use materials and equipment safely and cost-effectively.
  • Foster a positive environment for coworkers and patients and encourage teamwork.
  • Participate in professional development, and complete mandatory education and competencies.
