Horizon Pharmacy is an independent pharmacy that has been serving Sussex County and surrounding communities since 2021. Horizon Pharmacy is owned and operated by Horizon Health Services, Inc., a Federally Qualified Health Center (FQHC), that has been serving Sussex, Surry, Southampton, and surrounding communities for over 43 years.
Roles and Responsibilities :
Responsible for the evaluating, preparing, and accurate dispensing of medications. Reviews prescriptions for appropriateness, compatibility with current therapy, drug allergies, adverse drug effects, and / or duplication of therapy to ensure patients medication therapy is safe, effective, timely, and free from drug-related problems.Performs prescription verification of all prescriptions and provides technical guidance to pharmacy technicians.Maintains patient profiles and reviews for accuracy of data.Ensures compliance with all Federal 340B Drug Pricing Program requirements.Follows all pharmacy laws and regulations as set by the Virginia Board of Pharmacy.Submits, reviews, and handles all adjudicated prescription claims to ensure appropriateness of reimbursement, co-pay, and related financials and, if needed, follows up on all such claims deemed inappropriate or rejected.Recommends formulary alternatives, when appropriate.Develops and maintains good working relationships with physicians and other healthcare professionals in order to facilitate patient care and best serve customers.Serves patients / customers in a friendly, courteous, and professional manner to promote patient / customer satisfaction. Operates with a focus on excellent customer service.Interacts closely with individual patients in order to answer all questions and / or concerns regarding their medication(s). Provides medication counseling, as needed, both verbally and through proper use of literature.Monitors the overall performance of the pharmacy technicians, and collaborates with the pharmacy manager, to ensure compliance with regulatory and organizational policies, practices, and approved pharmaceutical dispensing processes.Maintains strict control and accountability of pharmacy inventory, including removal of expired and recalled drugs.Responsible for daily drug ordering and maintaining appropriate inventory levels. Ensures that the most effective and economical drugs are being used.Maintains all relevant information regarding drug recalls.Records all pharmacy dispensing errors upon identification, and reports errors to the Pharmacist-in-Charge and the HHS Chief Compliance Officer.Maintains controlled substance records, as required by law.Maintains an adequate inventory of narcotic drugs. Investigates narcotic discrepancies in a timely manner and reports discrepancies to the Pharmacist-in-Charge.Assists in the safeguarding against diversion of drugs.Assists with the OTC area, including determining product mix, ensuring stocking / replenishment, and responding to customer / facility requests for special order OTC items.Performs other miscellaneous job-related duties, as assigned.Qualifications :
Bachelors degree in pharmacy or PharmD (preferred) from an accredited school of pharmacy.Current, unrestricted license to practice pharmacy in the State of VirginiaBasic Life Support (BLS)A minimum of two years experience in retail pharmacyExperience with the Federal 340B Drug Pricing Program preferred
