Must-Have : EPIC experience; skilled in placement for SNF / IPR / LTAC / Hospice / Home Health; experience with care management skills including assessment, care coordination, and discharge planning

Description :

Registered Nurse - Case Manager needed for a dynamic healthcare environment.

The average census and case load is 99% capacity on most days.

The nurse will manage 14-16 patients depending on the unit assigned.

The ideal candidate will possess a minimum of 3-5 years of experience in Case Management.

First-time travelers will not be accepted for this position.

The patient population may include adults in Medical / Surgical, Cardiac, Ortho, and possibly Pediatrics.

Required experience includes skilled placement for various healthcare services, including behavioral health needs.

The position requires routine engagement with the homeless population.

Care management skills such as assessment, coordination, and discharge planning are essential.

Support within the department includes a Director, Managers, Chief Medical Officer, and a supportive utilization management team.

Peer case managers and social workers are also available for assistance.

The technology used includes an electronic medical record system that is required for the role, as well as referral management systems.

There may be potential floating to other units as needed.

Weekend rotation will depend on departmental needs.

Travelers will work on holidays unless previously approved for time off.

Scrub color is navy blue.

The department culture is supportive and teamwork-oriented, with managers who communicate effectively.

Educators provide support through changes, and seasoned staff are available with extensive knowledge.
