Registered Nurse – Case Manager – LBH - Sinai Hospital

Location : 2401 W Belvedere Ave, Baltimore, MD 21215

Unit : Case Management (1000-6620 CM)

Duration : 14 weeks

Positions : 1

Shift : Day 5x8-Hour (8 : 00 AM – 4 : 30 PM)

Weekend Coverage : 2 weekend day shifts per month

Scheduling : Block schedules (monthly schedule from 1st to last day)

Job Overview :

Manage acute care case management for a high-volume hospital (300+ beds). Responsible for discharge planning and coordination for diverse patient populations across multiple specialties and age groups.
