We are seeking a skilled and compassionate Registered Nurse to work one-on-one with our patients in their home environment. As a Patient Care Manager, you will establish and oversee all aspects of case-managing patient's care.

Key Responsibilities :

• Assume responsibility for assigned patient's care from admission through discharge by effectively executing patient's Plan of Care.
• Collaborate with the patient, family / caregiver(s), ordering physicians, other care team members, and community resources.
• Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.
• Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.
• Participate in on-call rotation as reasonably assigned.
