job_description.job_card.job_descriptionCollaborate with healthcare professionals during weekly team meetings. Provide rehab and manage medical needs in patients' homes. Administer medications and monitor their effects. Review and adjust care plans and medications as needed. Evaluate and support staff competency in direct care. Communicate patient updates to physicians and the team. Maintain patient records and report incidents as required. Promote patient independence and consumer choice in care. Medical, dental and vision Short and long-term disability Mileage Reimbursement Life insurance Paid time off 401(k) matching program Education / Tuition / CEU Reimbursement up to $4,000 Company provided Medbridge CEU Flexible Spending Account (FSA) Employee Assistance Program (EAP) Vendor discounts sign-on bonus applicable to external / new hires only Attends and actively participates in weekly team meetings Provides rehabilitation and coordinates medical needs for clients Medication Monitoring and Administration by oral medication or needle injections Orders, reviews, monitors, and discontinues medications as ordered by the physician Assists with keeping within assigned nursing budget Conducts assessments and approves plan of care and service delivery methods Performs competency and skills evaluations on employees who provide direct care Informs the physician and staff of changes in the client's medical or mental condition and needs Writes progress notes, summaries, and discharges Implements the treatment / rehabilitative activities and strategies Reports significant / major incidents or accidents and notifies proper authorities of circumstances or situations that present possible health hazards Follows consumer health plans and provides and promotes consumer choice and facilitate independence Assists in maintaining and updating nursing, pharmacy, infection control, and health and safety policies and procedures Other tasks as assigned Graduate of an accredited school of Nursing Current Registered Nurse licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire Bachelor's degree preferred, with one (1) year of home health experience preferred Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist
