Job Title : Occupational Therapist

Experience : Minimum years of experience required; new graduates are welcome.

License : Required

Certifications : Valid license verification required

Must-Have :

• SNF experience preferred.
• Covid test required 72 hours prior to start date with results issued 48 hours prior.
• Completion of credentialing checklist prior to submission.

Description :

A resume and license verification also need to be attached.

If these items are not completed and attached, the candidate will be declined.

Three references are required.

A background check covering county and federal criminal records for the past seven years is necessary.

A multi-state criminal search and nationwide sex offender search for the past seven years are also required.

HHS / OIG and GSA searches must be completed.

A social security number trace is required.

Submission of a one-step PPD or Quantiferon test or proof of a positive PPD and chest X-ray must be provided.

SAM search and a state police background check are necessary.

Completion of a mandated reporter form and sworn statement is required.
