Primary Care Advanced Practice Provider (APP)

Location : Mt. Carroll, IL

About the Position :

Hospital group is seeking a dedicated and patient-focused Advanced Practice Provider (APP) to join our busy outpatient clinic located in Mt. Carroll, Illinois.

APP experience preferred

Clinic-based role in a rural, community-focused setting

Busy practice environment with an established patient panel

Schedule : 4.5 days per week

Current team : 1 experienced APP on staff

Call : Some after-hours phone-only call required (minimal volume)

Compensation & Benefits :

Salary : Competitive base range of $120,000 $180,000 , plus production-based bonus potential
