Responsibilities
Coordinate patient care plans and ensure effective case management.
Monitor and evaluate patient progress and adjust care plans as needed.
Communicate effectively with patients and their families regarding care plans.
Collaborate with healthcare teams to optimize patient outcomes.
Maintain accurate and up-to-date patient records and documentation.
Ensure compliance with healthcare regulations and standards.
Identify and address barriers to patient care and recovery.
Develop and implement strategies to achieve case management goals.
Participate in professional development and continuous learning opportunities.
Utilize data and metrics to assess and improve case management processes.
Track and report on case management outcomes and performance metrics.
Contribute to team meetings and share insights for process improvements.
Experience / Qualifications
Valid RN license in California with case management experience.
Strong communication skills to interact with patients and families effectively.
Experience in developing and adjusting patient care plans.
Ability to meet targets and work effectively in a results-driven environment.
Proficiency in electronic health records and documentation.
Ability to collaborate with interdisciplinary healthcare teams.
Strong analytical skills to utilize data in process improvement.
Experience in identifying and addressing barriers to patient care.
Knowledge of healthcare regulations and compliance standards.
