✅ What You'll Do as a Home Health LPN

• Provide direct nursing care to patients in their homes across assigned territories
• Administer medications, wound care, IV therapy, and other treatments
• Educate patients and caregivers on disease processes, medications, and home care techniques
• Collaborate closely with RN Case Managers and the full interdisciplinary care team
• Ensure accurate and timely clinical documentation
