Pennsylvania Licensed TeleCare Nurse

A company is looking for a Primary Care TeleCare Nurse.

Key Responsibilities

Evaluate and assess healthcare needs of patients through telephonic evaluation of reported signs and symptoms

Deliver, manage, and coordinate care and services within Ambulatory Care Nursing

Ensure continuity of care through communication with the patient's healthcare team

Required Qualifications, Training, and Education

Minimum of 3 years recent clinical experience in critical care, emergency department, home health, or physician practice nursing

At least 1 year of telephone triage experience is strongly preferred

Associates Degree in Nursing from an accredited school is required

BSN is strongly preferred

Current PA RN license or a multi-state license through the Nursing Licensure Compact
