Job Summary

The Service Coordinator at Whispering Pines Senior Village is responsible for connecting residents with services and supports that enable them to maintain independence, improve quality of life, and remain safely housed. This position provides service coordination, advocacy, and program facilitation to help residents access healthcare, supportive services, and community resources. The Service Coordinator ensures all duties are carried out in a manner that respects resident privacy, promotes well-being, and complies with applicable laws and organizational policies.

Education & Experience

  • Bachelor's degree in Social Work, Psychology, Gerontology, Sociology, Counseling, Public Health, or related field preferred; equivalent relevant experience in case management may substitute.
  • Minimum of two years of experience providing service coordination, case management, or direct resident support.
  • Experience conducting needs assessments and connecting individuals to appropriate community resources.
  • Experience working directly with older adults or individuals with disabilities.
  • Experience in aging services, disability supports, mental health, or substance use services preferred.
  • Experience utilizing electronic systems, including experience with Microsoft Office, data entry systems, and electronic recordkeeping.

Essential Job Duties

  • Provide general case management, including intake and referrals, and conduct formal case management when community services are not available.
  • Coordinate supportive services that enable residents to age in place and delay or avoid higher levels of care.
  • Routinely assess service needs in response to changing circumstances, including when residents return from hospitalization or rehabilitation.
  • Conduct annual health and wellness assessments to evaluate the physical, mental, functional, and social status of residents.
  • Develop individualized service plans guided by assessments, data, and observations to address identified needs and interests.
  • Establish links with agencies and community service providers to identify cost-effective, flexible, and creative service options.
  • Refer and connect residents to providers such as case management, personal assistance, homemaker services, meal delivery, transportation, counseling, preventive screenings, and legal advocacy.
  • Monitor delivery of resident services to ensure they are appropriate, timely, affordable, and satisfactory.
  • Build partnerships with public agencies, social service organizations, hospitals, health systems, and primary care providers to strengthen service capacity.
  • Organize and coordinate on-site programs, events, and activities that promote health, wellness, and resident engagement.
  • Engage and motivate residents, including socially isolated individuals, to participate in programs that support health and well-being.
  • Educate residents on available services, application procedures, and client rights, and provide advocacy as appropriate.
  • Report suspected abuse situations to the appropriate agency in compliance with policy and law.
  • Maintain accurate and confidential resident records, including assessments, contacts, and service plans, in appropriate databases.
  • Prepare and submit required reports, pursue additional service resources, and participate in training, evaluation, and quality assurance activities to support program improvement.

  • Knowledge, Skills, & Abilities

  • Knowledge of case management practices, including assessment, service planning, and follow-up.
  • Knowledge of aging-related issues, disability services, and community-based supports relevant to resident needs.
  • Knowledge of federal and state benefit and entitlement programs, including eligibility and application processes.
  • Knowledge of confidentiality and privacy standards related to resident information.
  • Skilled in building and maintaining collaborative relationships with healthcare providers, community agencies, and service organizations.
  • Skilled in written and verbal communication, problem solving, and organizational tasks.
  • Skilled in use of electronic data systems, word processing, email, and internet-based research tools.
  • Ability to advocate for residents to secure necessary services and resolve barriers.
  • Ability to engage and motivate residents, including those who may be socially isolated, to participate in programs that support health and well-being.
  • Ability to work independently and exercise sound judgment, while also collaborating effectively as part of a team.
  • Ability to establish trust with residents, assist them in problem definition, and facilitate decision-making toward solutions.
  • Ability to maintain accurate records and prepare clear, timely reports.
  • Other Eligibility Requirements

  • Must complete a required pre-employment criminal background and reference check after a conditional offer of employment is made.
  • Must not be excluded from participation in federal healthcare programs, including but not limited to listings on the Office of Inspector General (OIG) List of Excluded Individuals / Entities and the System for Award Management (SAM) exclusion list.
  • Must be legally authorized to work in the United States without sponsorship.
  • Must have an active driver's license, current insurance, reliable transportation and ability to meet VOA Oregon's and VOA Oregon's insurer's driver standards.
  • Travel Requirement

    This position does not require regular travel as part of its primary job duties. Occasional travel may be necessary for training, meetings, or other business needs.

    This position does not have supervisory responsibilities.

    Physical Demands & Work Environment

    The physical demands described here are representative of those that must be met by an employee to successfully perform the essential duties of this job. While performing the duties of this position, the employee is regularly required to sit, stand, walk, use hands and fingers to operate tools and controls, and communicate effectively. The employee must occasionally lift and / or move up to 25 pounds.

    The work environment includes both an office setting and a residential living environment. The noise level is generally low to moderate.
