DUTIES / RESPONSIBILITIES-
- Provides primary medical care, including preventative services, as well as diagnosis and treatment of acute and chronic illness within licensed scope of practice and granted privileges for both pediatric and adult patients.
Includes physical examinations, ordering and interpretation of diagnostic tests and imaging, performing indicated medical procedures, prescribing medications, referral to specialty physicians and / or health resources as indicated
Ensures the delivery of all medical care is consistent with regulatory, accreditation, and professional standards, including clinical policies and quality initiativesParticipates in the review and development of clinical policies, protocols and standing ordersActively engages in compliance and quality improvement activities, including but not limited to, peer review, patient-centered medical home, UDS reporting, PDSA cycles, SOAP notes, other value-based care projectsAttends all scheduled provider and all-staff meetings and trainings, unless excused by CMOEnsures accuracy, timeliness, and appropriate confidentiality with patient electronic documentation per contractWorks evening and weekend hours as scheduled and shares after-hours call coverage with other providersConducts all scheduled clinic shifts in person at assigned clinic location (Northview Heights and / or Middle Street offices) and completes virtual visits as needed.Develops and ensure ongoing, positive relationships and collaboration with all health center staffMeets professional behavioral expectations for attendance, appearance, professionalism, communicationTrains, mentors and supervises students, trainees, and mid-levels as assignedWorks with a multidisciplinary teamIdentifies barriers to self-management plan and meeting goalsConsistently practices universal precautionsPotential exposure to biohazardous materialsProlonged sitting and / or standing and moving about throughout the workdayWillingness to participate in a minimum of 2 hours of community outreach per yearIs punctual and ready to participate in clinic care at start of each clinic sessionParticipates in morning patient care huddles, up to 10 minutes outside of patient care hoursComfort working with the NSCHC faith-based mission to provide care for all who request it, without judgementKNOWLEDGE / SKILLS / ABILITIES-

M.D. or D.O. from an accredited educational institutionLicensed to practice medicine in the State of PennsylvaniaMaintains requisite CME credits for area of specialtyBoard-certified / Board-eligibleActive DEA license requiredCurrent provider card in Basic Life Support (BLS)Clearances : Act 33, 34, 73Annual COVID vaccination encouragedAnnual influenza vaccine requiredMust obtain or provide proof of (within 14 days of hire) : PPD, Tdap, Hepatitis B immunity, MMR immunity, varicella immunityComplete self-attestation of fitness for duty
