The Role

We are seeking compassionate and skilled Physical Therapist Assistants to join our team- PRN to full-time opportunities available. You’ll work with patients in the Longmont, Firestone, Frederick, Berthoud, and Loveland areas , helping them regain mobility and improve their quality of life. This role is ideal for a therapist who enjoys building strong patient relationships and providing personalized, hands-on care.

Core Responsibilities :

  • Provide in-home physical therapy services based on the established Plan of Care
  • Document visits, observations, and client progress accurately and timely
  • Educate and support clients and families on safe practices and exercises
  • Communicate concerns or changes in condition to the Director of Clinical Services
  • Collaborate with the interdisciplinary care team and participate in client care meetings
  • Ensure a safe home environment and follow infection control policies

    Requirements :

    What Makes You a Great Fit

  • Licensed Physical Therapy Assistant in the state of Colorado
  • Graduate of an accredited PTA program (APTA approved)
  • CPR certified
  • Minimum of 6 months experience as a PTA (home health experience a plus!)
  • Strong communication, organization, and interpersonal skills
  • Reliable vehicle, valid driver’s license, and insurance
  • Passion for helping others reach their goals
