About The Position

Job Title : Physical Therapy Assistant

Profession : Rehabilitation Therapy

Duration : 16 weeks

Shift : Monday - Friday, Days

Hours per Shift : 32 guaranteed hours per week

Experience : Accepting new graduates; skilled nursing facility experience preferred

License : State License Verification required

Certifications : CPR certification required

Must-Have :

Resume, three references, negative drug screen results, and other background checks as outlined in the credentialing checklist.

Description :

Ensure prescreen is filled out and attached when submitting the candidate.

Resume and license verification must also be attached.

If these items are not completed and attached, the candidate will be declined.

Rehab Optima experience preferred.

Completion bonus offered for the assignment.

Review the credentialing checklist prior to submission.

Requirements include state license verification, resume, three references, county criminal check for all counties lived and worked in the past seven years, federal criminal check for seven years or more, multi-state criminal search for seven years or more, nationwide sex offender search for seven years or more, HHS / OIG search, GSA search, social security number trace, one step PPD or Quantiferon or proof of positive PPD and chest x-ray, SAM search, and CPR certification.
