LPN (Clinical Staff)- Remote

Arizona

M-F 8 am to 4pm

60k to 65k (DOE)

Primary focus : Routine clinical follow-ups, symptom checks, med rec support, education reinforcement.

• Monthly CCM outreach (telephonic / virtual) : Vitals / symptoms, adherence, self-management check, teach-back.
• Med reconciliation support, refill coordination; device use coaching(e.g. inhalers, home BP / SpO2, remote monitoring triage.
• Care plan updates under RN direction, standing orders / protocols as allowed
• Transitions of care : 48-72h follow-up calls, warm handoffs, to RN for Red Flags
• Documentation : timely notes, times loggings for CCM minutes; close the loop on tasks / referrals
• Typical Direct care time 65-75% of FTE

Required :

LPN License in the state of AZ
