As a Care Coordinator, you will : \n\nKey Responsibilities : \n
- Admit patients for hospice services as per physician orders.\n
- Assess and evaluate patient needs, establishing mutually agreed goals.\n
- Communicate patient status and required interventions to clinical and hospice physicians.\n
- Continuously update care plans, revising patient problems and goals during recertification or as changes occur.\n
- Conduct informational visits and obtain patient consent for hospice admission in accordance with office protocols.\n
- Utilize the 4Ms—What Matters to the patient, Medications, Mentation, and Mobility—to ensure Age-Friendly Care.\n\nRequired Skills and Experience : \n
- Current, unencumbered RN license in the state of practice, compliant with Nurse Licensure Compact (NLC) regulations.\n
- Valid driver's license with a good driving record.\n
- Proven ability to work independently in a field setting and make sound nursing judgments.\n
- Capability to assess patient needs and create tailored care plans effectively.
