Job Description :

Sunbelt Staffing is actively hiring an experienced Home Health RN for a 13-week assignment in vibrant Phoenix, AZ. This role involves managing a home health caseload, conducting skilled nursing visits, and using both OASIS and Homecare Homebase for documentation. Nurses who excel in autonomy and patient education will thrive in this position.

Key Responsibilities :

Provide in-home nursing care and clinical assessments

Accurately complete OASIS documentation

Utilize Homecare Homebase for charting

Educate patients and caregivers on care plans and medications

Collaborate with interdisciplinary team

Participate in scheduled on-call coverage

Qualifications :

Active RN license in Arizona

At least 2 years of home health experience

Proficiency in OASIS and Homecare Homebase

AHA BLS Certification

Valid drivers license, auto insurance, and reliable transportation

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
