Contract Details :
- Job Type : Contract (13 weeks), with possibility of extension
- Location : East Hartford, CT, and surrounding areas
- Schedule : Full-time, Monday-Friday, 8 : 00 am - 5 : 00 pm, occasional weekends and on-call (typically one weekend per month)
- Hours : Full-Time, Scheduled 40 hours per week - no guaranteed hours
- Compensation : Competitive hourly pay (negotiable based on experience and territory); paid hourly - not by visit; blended pay packages available for travelers; weekly pay

Conductcomprehensive patient assessments in home settings, ensuring a personalized and holistic approach to care.Utilize Oasis and Homecare Homebase for accurate documentation.Developindividualized care plans and implement skilled nursing interventions to enhance patients' well-being and support their health goals.Utilizeelectronic health records for accurate documentation and effective communication with the healthcare team.Administerprescribed medications, including injections, and provided education to patients and their families on proper medication management.Collaborateclosely with the interdisciplinary home health team, including physicians, therapists, and social workers, to ensure coordinated and high-quality care.Conductregular patient education sessions, emphasizing health management, disease prevention, and self-care practices.Manageand coordinate patient care, ensuring that all aspects of the care plan are implemented and adjusting the plan as necessary based on patient needs.Participatein regular team meetings to discuss patient progress, share insights, and contribute to the overall improvement of patient care.Senior National Healthcare Consultant
