Details :

Personal Care & Daily Living Support :

Provide support for mobility and physical transfers, ensuring comfort and safety.

Help clients with medication reminders and ensure proper medication administration.

Support with housekeeping duties, including cleaning, laundry, and organizing to maintain a safe and comfortable living environment.

Goal Setting & Skill Development :

Support individuals in reaching personal goals outlined in their Individualized Service Plans (ISPs), such as improving self-care, communication, and social skills.

Assist with the development of life skills, including budgeting, cooking, cleaning, and time management to promote greater independence.

Encourage clients to engage in social, recreational, and educational activities to enhance personal growth and community integration.

Behavioral Support :

Implement positive behavior support strategies to manage challenging behaviors and encourage constructive behavior.

Use de-escalation techniques when necessary and support individuals during moments of frustration or distress.

Work with the team to maintain and adapt behavioral intervention plans tailored to each individual's needs.

Socialization & Community Integration :

Promote participation in community activities such as shopping, attending events, sports, or volunteering, to build social connections and a sense of belonging.

Encourage and facilitate interactions with peers and community members, enhancing social skills and reducing isolation.

Accompany individuals on outings or appointments, providing emotional support and guidance in new or unfamiliar situations.

Documentation & Reporting :

Maintain accurate, up-to-date records of client activities, progress, and any changes in behavior or health status.

Report incidents or changes in client condition to supervisors, caregivers, or medical professionals promptly.

Ensure all client documentation is in compliance with HIPAA and other confidentiality regulations.

Health & Safety :

Monitor individuals for any health concerns, ensuring proper hygiene, diet, and physical health.

Respond appropriately in emergency situations, including medical issues or behavioral crises, while maintaining a calm and supportive environment.

Follow all safety protocols to ensure a safe environment for both clients and staff, including safe transportation and preventing accidents.

Collaboration & Teamwork :

Work as part of a team with other Direct Support Staff, supervisors, and professionals to meet the needs of individuals with developmental disabilities.

Participate in team meetings and contribute to the development of individualized care plans.

Support a family-centered approach, collaborating with family members to maintain continuity of care and community involvement.
