Licensed Practical Nurse (LPN) – Assisted Living Memory CAre

Delivering compassionate, high-quality care where it matters most — right in the patient’s home. We’re seeking a dedicated Licensed Practical Nurse (LPN) who is ready to make a real difference, one patient at a time.

What You’ll Do

Provide skilled nursing care and support to patients in their home environment

Coordinate care plans with the clinical team to ensure optimal outcomes

Monitor patient health, administer medications, and educate patients and families on continued care

Document visits accurately and in a timely manner

Maintain a positive, professional, and compassionate attitude in every interaction

What You’ll Need

Active and unrestricted LPN license in the state of Ohio.

Minimum 1 year of clinical experience (acute care, home health, or similar setting preferred)

Current CPR certification through the American Heart Association or American Red Cross

Strong communication and organizational skills

Reliable transportation for patient visits

A positive attitude and commitment to quality patient care
