Working independently on weekends (Saturday & Sunday, typically 8 AM-5 PM). Handling intake, documentation, and coordination with minimal handholding. Communicating with account executives, hospitals, and field staff to ensure smooth admissions. Using AXXESS EMR systems and juggling multiple priorities in real time. We trust our nurses - you’ll have autonomy and support. Our leadership is accessible, responsive, and actually listens. We’re small enough to be nimble, but big enough to offer stability. You’ll be part of a team that values clinical excellence and human connection. Current Texas LVN license (required). At least 1 year of recent experience in home health, intake, or case management (preferred). Solid understanding of Medicare, Medicaid, and private insurance guidelines. Strong communication, organizational, and customer service skills. Proficiency with AXXESS EMR systems and Microsoft Office.
