To To provide personal care, meal preparation, and light housekeeping assistance to clients.

Responsibilities

• Assist with the successful onboarding, training, mentoring and retention of paraprofessional staff.
• Provide hands-on personal care to patients, including bathing, mobility assistance, transfers, and range-of-motion exercises, following care plans and safety procedures.
• Assist with meal preparation, light housekeeping, and errands to maintain a safe, clean, and supportive home environment.
• Observe and report changes in patient condition to the Case Manager.
• Respond to emergencies according to agency procedures.
• Utilize agency-issued technology for documentation and communication.
• Achieve performance goals (KPIs) and complete required in-service trainings.
• Perform other duties as assigned to support patient care.

Qualifications

Physical Requirements

Must be able to perform the physical activities associated with the position, including bending, stretching, reaching, kneeling, climbing stairs, and moving / lifting up to 40 lbs. repeatedly; pushing, pulling, and transferring up to 100 lbs.; and completing written documentation and record-keeping.

Sensory Abilities specifically required :

Visual acuity required to read plan of care, order, etc. Hearing acuity require to perform all aspects of the job. Sufficient hand strength and dexterity to administer the care plan duties.

The referenced pay range represents the minimum and maximum compensation for this job family. Individual annual salaries / hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
