Description :

This position provides coordination and direction of home nursing care according to physician orders, based on agency policies and procedures.

The nursing process will be competently applied to ensure effective patient care.

Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

The role involves home visits and attendance at periodic office meetings.

Must have reliable transportation and a current driver's license, along with agency-required liability insurance.

Daily responsibilities include a patient load averaging 5-8 patients per day.

Each type of visit and activity, including documentation and travel, holds a different point value contributing to weekly productivity expectations of 30 points.

Types of cases include Start of Care, Wound Care, and Follow-up Visits.

Clinicians are expected to accept all patients assigned to their schedule, regardless of proximity to previous appointments.

All documentation must be completed within 24 hours of patient appointments.

There are no guaranteed hours; clinicians must meet productivity points to support their financial needs.

Mileage will be reimbursed at the IRS standard rate for travel between patient homes.

Home health or hospice experience is required, and individuals cannot currently be direct employees of any affiliated location when applying.

Must be willing to travel up to 75 miles from the assigned home clinic.

On-call availability varies based on location and profession.

All travelers must have recent experience with required charting systems.

The role may require occasional travel to care facilities.
