Home care experience required

Scope of Role :

Assist licensed Physical Therapists in providing patient care

Support patients with rehabilitation exercises, mobility, and functional activities

Follow treatment plans and document patient progress according to facility protocols

Collaborate with PTs and other healthcare professionals to ensure safe and effective care
