The Role :

As a Care Navigator, you will play a crucial role in supporting individuals with dementia and their families (dyads). This is a remote role and provides services via telehealth. You will provide guidance, assistance, and resources to help navigate the complex journey of dementia care, ensuring that each person receives the best possible care and support tailored to their unique needs.

Note the available shifts (all listed in Central time, please convert to your personal time zone) :

• Mon-Fri 9 : 30am - 6pm CT
• Mon-Fri 10 : 30am - 7pm CT

The pay range(s) below are provided in compliance with state specific laws. Pay ranges may be different in other locations. Exact compensation may vary based on skills, experience, and location.

Role : Range is $48,500 - $63,500 depending on experience
