What You’ll Do :

Act as an advocate and emotional support system for residents and their families.

Provide guidance on resident rights, care planning, and transitions of care.

Coordinate services and resources to enhance resident wellbeing — from counseling to arranging personal items or support services.

Serve as a key member of our interdisciplinary team, working closely with staff and family to address individual needs.

Support residents through grief, adjustment, and life changes with compassion and care.

Document social services needs and ensure individualized care plans reflect evolving goals.

What We’re Looking For :

A licensed Social Worker in good standing, or a degree in social work or related human services field (sociology, psychology, gerontology, etc.).

At least one year of supervised social work experience in a healthcare setting.

Someone who is patient, empathetic, a great communicator, and thrives in a fast-paced, resident-first environment.
