Base Pay : $23.76 - $35.65 / hour

JOB SUMMARY

The primary function of the LVN Access Nurse is to utilize the nursing process to provide support to the RN Coordinator and the clinicians in areas where functions of nursing and medicine overlap. Accurately administers medications, including immunizations, by all routes except IV. In triage scenarios (phone or in person) the LVN Access Nurse can gather pertinent patient information to consult with a licensed professional authorized for independent triage. The LVN Access Nurse are expected to demonstrate critical thinking skills, leadership and appropriate role modeling.

JOB DUTIES AND RESPONSIBILITIES

Utilizes elements of the nursing process in data collection and documentation for assessment purposes, in contributing to the plan of care, in assisting in the implementation of the plan of care and in providing information to the clinician or RN for evaluation purposes.

  • Initiates venipuncture with clinician’s order for the purpose of IV fluid administration (IV Certified LVN only).
  • Performs urinary bladder catheterization with clinician’s order.
  • Counts and signs for controlled substances according to clinic policy.
  • Assists in the performance of face to face or telephone triage using written triage protocols; consults with the supervising practitioner and informs the clinician of the advice / instructions given or actions taken.
  • Accepts and correctly transcribes verbal or telephone orders from clinicians.
  • Calls in new prescriptions and prescription with changes in dose, duration, method of administration or frequency with a provider order.
  • Functions as Team Leader when assigned.
  • Performs medication refills according to Refill Protocol.

Assisting the Clinician and Daily Activities :

  • Prepares the patient and assists the clinician with exams and procedures when needed.
  • Administers injections and diagnostic test / referrals as ordered, including immunizations, completing lab and x-ray requisitions and other services ordered by the clinician or according to center protocols.
  • Performs age-appropriate screening as needed or as directed by clinician.
  • Performs accurate nursing assessment of patients admitted for urgent care and initiates emergency measures as appropriate (consultation of RN, FNP, PA, MD, or DO required).
  • Ensures end of shift tasks are completed appropriately.
  • Assures efficient patient flow by ongoing assessment of clinician schedule and working with other staff to minimize patient waiting time.
  • Follows and teaches Standardized Precautions in compliance with OSHA and SCHC standards
  • Schedule’s patients appropriately.
  • Alerts Center Manager or designated person to needed supplies / equipment.
  • Reports any safety hazards.
  • Other duties as assigned.
  • KNOWLEDGE, SKILLS, AND ABILITIES

  • Knowledge of nursing process, scope of practice and standards of care.
  • Ability to pass a basic written medication exam.
  • Passes Medication Refill exam.
  • Basic Computer proficiency in Windows environment.
  • Typing 35 WPM or greater.
  • EDUCATION & EXPERIENCE

  • California LVN license upon hire and maintain current
  • CPR certification upon hire and maintain current
  • Minimum of one year’s experience in acute care nursing.
  • PREFERRED QUALIFICATIONS

  • Previous experience working in an outpatient clinic or doctor’s office.
  • Typing 45 WPM or greater.
