Key functions of SNF physical therapy

Evaluation and assessment :

Physical therapists assess a patient's safety, mobility, and current level of function upon arrival.

Treatment planning :

They create individualized treatment plans that may include exercises, stretches, and other therapeutic techniques like aquatic therapy.

Restoring function :

Therapists work to improve strength, coordination, balance, and range of motion.

Pain management :

Treatments are used to reduce pain and inflammation.

Preventing complications :

A key goal is to prevent falls and hospital readmissions.

Education and communication :

Therapists educate patients about their care plan and communicate with the entire care team, including nurses, other therapists, and doctors, to ensure a coordinated approach.

Transition planning :

Physical therapy is crucial for helping patients prepare to return home or to a new living situation.

Long-term care :

Therapy is also used for residents with chronic conditions like arthritis or Parkinson's disease to help them maintain function and independence.

Pay Packages are all inclusive, built on 40 HRS / 13 weeks and subject to confirmation and other fees not defined by facility.\n\nPer Diems (Housing and Meals) will be determined prior to submission and will be based on MAX allowed per GSA.
