Seeking a Home Health Physical Therapist to provide case management and direct patient care in the home setting.

Responsibilities include evaluating patient function, developing and revising care plans, administering therapy modalities, documenting progress, and collaborating with physicians, families, and the care team.

Strong communication, clinical skills, and ability to work independently in a team environment required.

Required : NYS OT License

BLS (AHA ONLY)

Home Health Experience within the last year

1 year of PT experience in a home health setting
