DEFINITION / PRIMARY FUNCTION
Coordinated team-based care to provide health services to individuals, through effective partnerships with patients, their caregivers / families, community resources and their physician. Facilitates a "shared goal model" within and across setting to achieve coordinated high quality preventive care that is patient and family-centered. The Licensed Practical Nurse supports the nursing process and functions under the direction of the Medical Director, Nurse Practitioner and / or Professional Registered Nurse.
POSITION QUALIFICATIONS (Minimum qualifications are required unless stated otherwise.)
Experience :
Has a thorough knowledge of nursing principle and procedures.
Experience in handling multiple telephone lines and excellent clerical / organizational skills required.
Experience in specimen testing and physician office experience preferred.
Education and Training :
Graduate of an accredited School of Practical Nursing.
License, Certification & Registration :
LPN - Licensed Practical Nurses
BLS
Other Requirements :
Demonstrates continuing education in accordance with State regulation.
Data entry skills preferred.
Working knowledge of Windows and PC applications required.
Ability to work independently and take direction.
Self-motivated and has the ability to be an independent and flexible in a team framework.
Excellent oral and written communication skills.
Demonstrates ability to handle confidential and sensitive information.
