A company is looking for a Care Coordinator.
Key Responsibilities
Engage with prospective clients and record interactions in the CRM system, making 60-100 outbound calls daily
Convey client preferences to the care team for personalized care planning
Assist clients in completing psychosocial assessments and developing actionable care plans
Required Qualifications
Active California LVN license, Medical Assistant certificate, or Bachelor's degree in a related field with 2+ years of direct healthcare experience
Experience working with vulnerable populations is preferred
Comfortable with technology, including video conferencing and electronic health records
Prior experience in a remote role with a secure workspace
Bilingual (English / Spanish) preferred
