Pathways Health Partners is an Accountable Care Organization (ACO) on the forefront of helping independent providers make the transition to Value-Based Care.  Pathways operate a Medicare REACH ACO, a Medicare Advantage MSO, a Commercial MSO responsible for approximately 16,000 patients throughout North-West / Central Florida.  Our service area is The Villages, down to St. Petersburg, across to Mount Dora, and areas in between.  Pathways also operates a robust hospital medicine group, manages several of its affiliated medical practices, and an insurance agency.

Job Summary

As a Registered Nurse in our GUIDE program with Pathways Health Partners, LLC., you will play a crucial role in providing high-quality patient care in various settings, including home care and assisted living facilities. Your primary responsibility will be to assess patients' health status, implement care plans, and ensure compliance with medical guidelines. You will work closely with patients and their families to educate them about health management and treatment options, fostering a supportive environment. Additionally, you will be responsible for wound care, pain management, and monitoring patients' progress to ensure optimal recovery. Ultimately, your contributions will significantly enhance the quality of life for our patients and support their overall well-being.

Role and Responsibilities :

Specific :

• Conduct thorough evaluations of patients' health conditions and document findings.
• Data Analytics
• Develop and implement individualized care plans in collaboration with healthcare teams.
• Respite Utilization Tracking
• Administer medications and treatments as prescribed, ensuring adherence to medical compliance.
• Provide education and support to patients and their families regarding health management and care options.
• Monitor patients' progress and report any changes to the supervising healthcare provider.
• General Building Procedures
• Referrals and Control Audits

Skills :

The required skills for this role, such as evaluating patients and developing care plans, are essential for ensuring that each patient receives personalized and effective treatment. Your ability to communicate with patients and family members will enhance their understanding of medical compliance and care options, fostering a collaborative approach to health management. Skills in wound care and pain management will be utilized daily as you provide hands-on care and monitor patients' recovery.
