JOB OVERVIEW

Job Title : Family Medicine / Internal Medicine Physician
Job Type : Permanent – Full-Time
Location : Hammond, IN
Service Setting : Outpatient
Schedule : Monday – Friday, standard clinic hours
On-Call Expectations : 1 : 4 call rotation responsibilities (hospitalist program available)
Start Date : Negotiable; credentialing required (90–120 days)

PATIENT POPULATION & SCOPE

Patient Demographics : All ages – pediatrics, adults, geriatrics

Average Daily Volume / Census : Outpatient clinic-based caseload

Admissions & Rounding : Not required; hospitalist team available

Consults & Follow-Ups : Yes

Case Mix / Clinical Focus Areas : Primary care, preventive medicine, chronic disease management, wellness

FACILITY & TEAM DETAILS

Beds in Department / Unit : Outpatient-based practice

EMR System : Epic

Specialty Backup : Multispecialty support throughout the system

Support Staff : Nurse practitioner and ancillary support available

Reason for Opening : Program growth and expansion

CREDENTIALING & COMPLIANCE

Board Requirements : BC / BE in Family Medicine or Internal Medicine

DEA & Certifications : Active DEA; BLS / ACLS required

Credentialing Timeline : 90–120 days

REQUIRED SKILLS & QUALIFICATIONS

Board Status : Board Certified or Board Eligible in Family Medicine or Internal Medicine

State License Required : Eligible for Indiana medical license

Certifications : BLS and ACLS required

Experience : Outpatient practice experience preferred; new graduates considered

Additional Requirements : EMR proficiency (Epic); collaborative, team-based care

DUTIES & RESPONSIBILITIES

Deliver comprehensive primary care services in an outpatient setting

Provide preventive, acute, and chronic disease management

Collaborate with nurse practitioners, specialists, and hospitalist teams as needed

Participate in a 1 : 4 outpatient call rotation

Contribute to quality improvement initiatives and community-based care delivery
