MINIMUM # of SHIFTS ACCEPTABLE 7

CERTIFICATION REQUIREMENTS - Will accept ABEM, AOBEM, ABFM, ABIM, ABPS or other BCs? FNP - ANCC, AANP or other BCs? ANCC / AANP for NP's; NCCPA for PA's; ABEM, AOBEM, ABFM, ABIM for physicians

Will the facility accept alternative boards? ABPS? No-they would need to petition the board for approval

RESIDENCY TRAINED REQUIREMENTS Must be boarded within 5 years of completing residency

ATLS, ACLS, PALS, BALS Required? PALS needed for Pediatric Hospitalist only; ACLS needed for NP's; ACLS needed for Physicians that are requesting moderate sedation

FLU OR OTHER IMMUNIZATIONS REQUIRED? WAIVER CONSIDERED? Current Flu, Current PPD, Current Physical, Titers for MMR, Hep B and Varicella. Waiver for flu only

PRIVILEGING TIMEFRAME 45-60 days

EXPEDITED LOCUM PRIVILEGES? No

ONBOARDING REQUIREMENTS (Orientation / EMR training) Current CV, SS card, Driver's Lic, NY Infection Control Certificate, ACLS for NP's, ACLS for physicians requesting moderate sedation, diploma, residency certificate, board certificate, CME's for past 2 years, case logs from past 2 years, Titers for MMR, Hep B and Varicella, current flu, current PPD, current physical, NY medical license, NY DEA, color photo, Current N95 fit test, COI's for past 10 years, time gap explanation for over 30 days, malpractice explanations, Self NPDB report. Epic Inpatient Training, which is in-person. It usually takes place on the first Tuesday of the month, after the Credentialing Committee meeting.
