Position Overview

HumanEdge Health is seeking a Travel Physical Therapist (PT) for a Long Term Care (LTC) facility in Childress, TX. The PT evaluates, diagnoses, and treats patients with functional impairments to restore mobility, reduce pain, and prevent disability. The therapist develops individualized treatment plans with patient-centered goals, collaborates with interdisciplinary teams, and ensures safe, high-quality care for residents.

Essential Functions

Perform patient evaluations through history-taking, systems review, and administration of tests and measures.

Establish individualized treatment plans addressing specific impairments and aligned with physician orders.

Reassess patient progress and modify treatment plans accordingly.

Initiate and manage discharge planning from the first visit through treatment completion.

Supervise and delegate to ancillary staff and students according to state laws and department policy.

Document patient care accurately, clearly, and within required timeframes, meeting accreditation and insurance standards.

Communicate effectively with patients, families, caregivers, and the healthcare team using AIDET principles.

Assess and provide patient and caregiver education as appropriate.

Participate in professional development through workshops, in-services, and current literature.

Engage in quality improvement initiatives and maintain a safe environment for patients, visitors, and staff.

Function within scope of practice as defined by the state’s practice act.

Perform other duties as assigned.

Submission Requirements

Minimum 2 years PT experience in a Long Term Care setting – Required

Active TX license or compact state license – Required
