Program Director, Care Coordination

Saint Joseph's Medical Center in Yonkers, New York is seeking an experienced Director for our Health Home Care Coordination Program. This program provides in-person, mobile and telephonic support services to adults with mental illness and other health challenges. Our Care Coordinators develop personalized care plans with each client with the goal of supporting independence in the community and connection to healthcare services and other community resources.

The Program Director manages the daily operation of the program and provides direct supervision to three Team Leaders and the Program Secretary. Other responsibilities include :

• Ensure compliance with Health Home and Medicaid billing and documentation standards.
• Collaborate with the County Department of Community Mental Health on referrals and AOT cases.
• Collaborate with hospital and community-based referral sources and service providers.
• As a member of the hospital's management team, ensure that the program consistently meets quality, budget and regulatory standards.

Requirements : Master's degree in social work or a related field with at least five years of experience working with adults with serious mental illness, and at least two years of experience in a supervisory role. in an outpatient mental health setting. A NYS license in social work or a related field is preferred.

Pay : $90-100k
