Job Description

This position involves providing and directing provisions of nursing care to patients in their homes, as prescribed by the physician and in compliance with applicable laws and regulations.

You will be responsible for coordinating total plan of care with other healthcare professionals involved in care, and helping to achieve and maintain continuity of patient care.

The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment.

Required Skills and Qualifications

• Current RN licensure in the state of Kentucky
• Current CPR certification
• A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
