HOME HEALTH – SPEECH LANGUAGE PATHOLOGIST  We have a great opportunity for a  Part-Time  Home Health Speech Language Pathologist in the Muncie / Alexandria, Indiana area . 1 :

• 1 Patient Care Flexibility to Create Your Own Schedule Mileage Reimbursement Weekend Hours Available, if desired Service Area – Muncie, Alexandria, and surrounding areas Monitor carefully the patient status and progress toward goals, as well as compliance with physician’s orders.
• Responsible for educating patients and caregivers, while preparing the patient / caregiver for discharge as appropriate.
• Job Responsibilities Provide skilled speech language pathology in accordance with the physician’s plan of treatment.
• Observe, record, and report the client’s reaction to treatment and any changes in the client’s condition to the physician, DON / Clinical Manager, according to policy.
• Develop, implement and revise the client’s plan of care.
• Promote coordination of care through interdisciplinary case conferences and communication with physician and field staff.
• Perform therapy interventions utilizing standard speech language pathology techniques and skills as appropriate for the condition of the patient.
• Education, Licensure(s) and Qualifications .   Previous Home Health experience is preferred but not required.
• Must be currently licensed to practice in the state of Indiana Must be CPR certified Current Drivers License, vehicle insurance and access to a dependable vehicle Equal Opportunity Employer Powered by JazzHR
