Houston, TX 77074 Pharmacist Per Diem - Job Overview Schedule :

  • Key Responsibilities Interpret, clarify, and transcribe medication orders with precision and timeliness.
  • Ensure medication appropriateness considering diagnosis, weight, allergies, and interactions.
  • Maintain compliance with all regulatory authorities governing pharmacy practice.
  • Review patient profiles for appropriate drug selection and dosage.
  • Conduct drug utilization reviews to prevent drug duplication and interactions.
  • Document adverse drug reactions and interactions effectively.
  • Dispense medications and maintain accurate inventory records.
  • Promote evidence-based medicine by staying informed on new developments.
  • Advise patients on health and illness prevention, including family planning services.
  • Provide comprehensive drug information and education to patients and healthcare providers.
  • Minimum Qualifications Graduate of an accredited college / university of pharmacy (Pharm.
  • D or B.S. with equivalent experience).
  • Certification in immunization delivery, smoking cessation, and naloxone (or eligibility).
  • Valid license from the Board of Pharmacy of the State of Texas.
  • Valid Texas Pharmacist Preceptor license.
  • Maintain First Aid / CPR certification.
  • Participation in Medicare and Medicaid Programs and other reimbursement programs. 2+ years of healthcare experience required; post-graduate pharmacy residency preferred. 1+ years of community pharmacy experience preferred. 1+ years of medication adherence counseling experience preferred.
  • Experience in HIV / AIDs, Geriatrics, and / or Pediatrics preferred.
