What You’ll Do : Evaluate, develop, and implement individualized treatment plans for residents.

Work closely with an interdisciplinary team to improve patients' mobility and daily living skills.

Educate patients, families, and caregivers on adaptive techniques and therapeutic exercises.

Document patient progress and adjust treatment plans as needed.

Ensure compliance with facility and state guidelines.

What We’re Looking For : Licensed Occupational Therapist in Delaware (or eligibility to obtain).

Experience in a SNF setting is a plus but not required.

Strong communication and teamwork skills.

Passion for patient care and rehabilitation.
