Overview : Provides support to the social work team. Maintains a modest caseload, providing direct patient care. Works closely with the Interdisciplinary team to meet identified patient / caregivers needs. Assists in developing and coordinating social work services across the organization.
Required Experience : Active LMSW or LCSW Social Worker license. 3 years of experience in hospice or related clinical field; management experience preferred. Knowledge of end-of-life care, hospice philosophy, and grief issues. Knowledge of resources within the community to meet the needs of culturally diverse groups.
SPECIFIC DUTIES AND POSITION REQUIREMENTS
- Provides direction, guidance, and support to all members of the social work team, especially on cases with complex psychosocial needs.
- Oversees the supervision and development of social work staff and interns, collaborating with the Director of Patient Support Services and Patient Support Services Coordinator to ensure team members have the proper resources needed to fulfill their job duties.
- Holds regular one-on-one meetings with direct care social workers for support and guidance.
- Runs team meetings in a meaningful manner.
- Makes recommendations on staffing needs, employee hires, transfers, and similar actions.
- Maintains collaborative relationships with other departments to ensure the ongoing delivery of quality care.
- Maintains a modest caseload in the area of highest need, providing direct patient care.
- Provides backup assistance for direct care social workers and bereavement counselor(s).
- Provides support to the social work team on issues related to crisis intervention and referrals for complicated psychosocial and mental health needs.
- Serves as backup for payroll tasks related to management of timekeeping, mileage, etc.
- Represents Circle of Life and actively participates in community events.
- Maintains current and relevant knowledge of changes in the Social Work field.
- Demonstrates compassion, empathy, and patience when interacting with patients, families, co-workers, and members of the public.
- Represents Circle of Life to the community in a positive manner.
- Demonstrates flexibility, versatility and a positive attitude in integrating additional duties.
- Performs other duties as assigned.
